Procedure Overview

Eyelid surgery, or blepharoplasty, is a straightforward procedure that typically takes between 30 minutes to 1 hour to complete.

It is usually performed under intravenous sedation, ensuring the patient remains comfortable and relaxed throughout the operation. Most patients are able to return home the same day.

• Upper Blepharoplasty

The procedure begins with upper eyelid surgery which addresses sagging or wrinkling of the eyelids. The surgeon removes excess skin and restores the natural eyelid fold. Incisions are made along the natural creases, ensuring that no visible traces of surgery remain after healing.

• Lower Blepharoplasty

Lower eyelid surgery targets puffiness and “bags” under the eyes. The surgeon carefully excises the fat pads causing the swelling and, if needed, removes any excess skin. In some cases, muscle fixation is performed to prevent lower eyelid laxity, ensuring that the lower lid remains firm and secure. For a transconjunctival approach, incisions are made along the mucous membrane of the lower eyelid, leaving no visible scars after recovery. This method is most suitable for younger patients, typically under 40, who have eyelid puffiness but no significant skin laxity.

• Additional Procedures: Fat Redistribution and Fat-Preserving Blepharoplasty

An advanced technique, known as fat-preserving blepharoplasty, may also be used. In this procedure, the surgeon redistributes fat tissue to smooth the transition from the lower eyelid to the midface as well as camouflage the nasolacrimal groove. This technique helps avoid the “hollow” eye effect, providing a more natural and rejuvenated appearance.

For patients with significant excess skin and wrinkles around both the upper and lower eyelids, these procedures can be performed simultaneously. The surgeon removes the excess tissue and repositions the skin, ensuring a seamless and refreshed result.

Recovery

Blepharoplasty is a minimally invasive procedure, and most patients experience a smooth and relatively quick recovery. In most cases, patients return home the same day or in some situations, the day after surgery.

  • Sutures are typically removed 5–7 days after surgery in a follow-up visit.
  • Steri-Strips are placed over the incision lines immediately after the procedure to support healing and are removed along with the sutures.
  • The most noticeable bruising and swelling occur on days 2 to 3, after which the healing process progresses quickly.
  • Stitches require minimal care at home and can be gently cleaned with a prescribed antiseptic solution to prevent infection and support healing.

To ensure the best result and avoid complications, patients are advised to follow these general postoperative guidelines

Avoid direct sun exposure for at least 4–6 weeks. Sunglasses are recommended outdoors.

Refrain from physical exercise, including gym workouts, running, and weight lifting for at least two weeks.

Do not use sunbeds or visit saunas or steam rooms during the recovery period.

Avoid swimming pools and hot tubs as they may increase the risk of infection.

Sleep with your head elevated for the first few nights to minimize swelling.

Following these instructions will help ensure a speedy recovery and the most aesthetically pleasing results. Always follow your surgeon’s individualized recommendations for post-operative care.

Why is BLEPHAROPLASTY a Leader in Rejuvenation?

  • Improved Vision
    Functional blepharoplasty improves vision.

  • Almost Unnoticeable
In transconjunctival blepharoplasty, the incision is made on the inner side of the eyelid, allowing for no visible signs of surgery post-healing.

  • One-Day Surgery
Eyelid surgery is usually performed within 30 minutes and does not require hospitalization.

  • Adaptive Anesthesia
Anesthesia with minimal risks, natural sleep, and quick awakening.

  • Quick Recovery
A week after removing the stitches, you can already use eye cosmetics. Swelling disappears within two weeks, and complete healing occurs after approximately four months.

  • Long-Lasting Results
The effect of blepharoplasty lasts for about 15 years and in some cases, even a lifetime.

Contraindications

While blepharoplasty is a safe and effective procedure, there are certain conditions that might make it unsuitable for some patients. The following are contraindications for undergoing eyelid surgery:

1. Active Infectious Diseases

Patients with active infections of the eyes, skin, or the upper respiratory tract are not candidates for blepharoplasty since such infection can compromise healing and increase the risk of complications.

Individuals with dry eye syndrome or insufficient tear production may experience discomfort and/or difficulties with healing post-surgery.

Patients with serious eye conditions, such as glaucoma, cataracts or retinal diseases, for example, may not be suitable for blepharoplasty as these conditions can affect the surgical outcome.

Individuals with severe cardiovascular issues, including uncontrolled hypertension, recent myocardial infarction, or arrhythmias, may face higher surgical risks and complications during the procedure.

Blepharoplasty is not recommended within the first 12 months after LASIK or similar procedures since the ocular surface and tear film must stabilize before undergoing eyelid surgery.

Patients with bleeding disorders, such as hemophilia, or those on anticoagulant medications may be at increased risk of excessive bleeding during and after surgery. This requires careful case-by-case consideration before proceeding with the procedure.

While blepharoplasty can be performed on older patients, individuals over the age of 75 may face a higher risk of complications and a slower recovery. A thorough medical evaluation is required to assess the suitability for surgery.

Conditions such as facial nerve paralysis or other neurological disorders affecting the muscles around the eyes may prevent proper healing and affect the results of the surgery.

Patients with uncontrolled systemic conditions, such as diabetes, thyroid disorders, or other health conditions that can interfere with healing, should consult with their primary care provider before considering eyelid surgery.
